Worker Information Form

In November the office will be sending out a packet of information that all members will be required to fill out and return. The packet will provide you with concise information about our Job Referral Procedures for Theatrical, Motion Picture and Commercial work. You will receive along with this, descriptions of Skills, Jobs and Classifications for all categories.

After going over this information, you will be required to fill out the Worker Information Form. This form will be used by the office to make sure that we have a correct understanding of your skills, experience and classification. You will also need to provide a resume (for some of you that remember our original ‘Skills’ sheet this replaces that form for better administration).

The form is divided into sections that chart out skills, equipment and professional experience. As well as providing basic contact information. The forms will need to be returned to the office no later than December 15th.
