If it has been awhile since you updated your tax forms please take the time to do so. If there’s a problem the office can be very helpful in getting you paid and on time if we have updated forms. W-9’s: Some employers are asking to have these forms filled when charging a kit rental. If you need copies the office can either fax or email these to you. Or you can download them from the "Forms" page in the "Members Only" menu right here on the website!
